Likely causes

What 011 tells you, and when it appears
Roku states that error code 011 indicates the software update has failed because your Roku device is unable to connect to the Roku Software Update Server. The distinguishing feature is when it happens: during initial setup, as the device attempts to download and install the latest software before it will let you go any further.
That timing changes what the error feels like. There is no home screen to go back to and no apps to test with, so the device looks more broken than it is. It is not — the setup process is simply refusing to continue until it has software it cannot currently fetch.
It is closely related to the numbered update failures a working Roku shows, and the underlying story is the same: the device cannot reach Roku's servers. If you are seeing this on a device that is already set up, the update-error sequence is the better page.
Roku's steps
Roku's first instruction is to wait a few minutes and select Try again. On a brand-new device that is genuinely worth doing more than once, because the alternative explanations all take longer to check.
Its second is to visit support.roku.com to check for service alerts indicating current problems. That step is easy to skip and it is the one that saves an evening: if Roku's own servers are having a bad day, no amount of restarting your router will change the outcome, and the only sensible response is to wait.
Third, if the device shows that it is disconnected from the internet, follow the reconnection process for your home network — and if the wireless connection keeps failing, Roku points to its own wireless network troubleshooting. A device that joined a network during setup but cannot reach anything through it is the most common local cause.
Proving where the fault is
Two checks separate the possibilities quickly. Open a website on a phone connected to the same wireless network: if that fails too, the network is the problem and the Roku is an innocent party. If it works, the network is fine and the device or Roku's servers are the remaining candidates.
If setup will not complete over Wi-Fi at all, an Ethernet cable on models with a port, or a different network entirely, is the decisive test. Completing setup on another connection tells you the device is sound and sends you back to your home network with a much narrower problem to solve.
